Older Adult Rehabilitation

Getting out of a chair, walking further or turning may start to feel difficult. An assessment helps identify what to work on. We plan rehabilitation around health conditions, current ability and daily routines, with carers involved where helpful.

Who this is for

  • Increasing difficulty standing up, walking or using stairs
  • Feeling unsteady, worried about falling, or having fallen before
  • Reduced strength and movement after illness or a long period of inactivity

What we work on with you

Assess everyday tasks

Review getting up, walking, balance and relevant health information.

Practise at a suitable level

Choose strength and balance exercises, adapting positions and aids to suit the person.

Include carers

Discuss home exercise support and everyday precautions, including assisting with standing and walking. Consider the carer’s own movement and handling to reduce injury risk, and changes at home to reduce the older person’s risk of falling.

What to expect at your first visit

  1. Discuss and assess

Start with your health history, difficulties and goals.

  2. Agree your care

Discuss the findings and an appropriate plan before starting care.

  3. Plan what comes next

Talk through practice, activity adjustments and follow-up.

Before your visit

Bring a medicine list, health information and any walking aid you use. A carer can join the discussion.

Questions before booking

Have another question? Ask the team before booking.

Can I attend with an existing health condition?

Often yes, but tell us about your conditions, medicines and medical advice so suitability and exercise adjustments can be considered.

Where should I start after a fall?

An assessment should consider possible causes and risks. Falls can involve more than strength or balance alone.

How is this different from senior exercise?

Rehabilitation focuses on functional difficulties and recovery. Senior exercise focuses on fitness and activity according to readiness.

What does it cost?

Ask the team about current fees, inclusions and appointment duration before confirming a booking.
