Do they speak English?
Worth settling before you travel across town. Physiotherapy depends heavily on communication - describing where it hurts, understanding what the therapist found, and following home exercises correctly. If any of that gets lost, the treatment loses much of its value.

Is it a physiotherapy clinic, or a massage shop?
In Bangkok the line can look blurry from the street. The practical test is what happens in the first fifteen minutes.
A physiotherapy clinic starts with an assessment - history, movement testing, hands-on examination - before anyone touches you with treatment in mind. A massage shop starts with the massage. Both have their place, but only one is designed to work out why something hurts.
Physiotherapy in Thailand is a licensed profession regulated by the Physical Therapy Council of Thailand. It's reasonable to ask whether the person treating you is a registered physical therapist.
Do they explain what they found, and what it will cost?
You should know two things before treatment starts: what the therapist thinks is going on, and what this is going to cost. A clinic that can't or won't tell you either is a clinic to be careful with.
Ask for a rough number of sessions too. Not a promise - nobody can promise that - but an informed estimate after an assessment is reasonable to expect.
Do they treat with exercise, or only hands-on and machines?
Manual therapy and modalities can settle symptoms in the short term. But if you want the problem to stay away, strength and movement usually need to change too.
Can you realistically get there more than once?
The most underrated factor. Physiotherapy is rarely a one-visit fix. A brilliant clinic you can only reach once a month will do less for you than a good one you can reach weekly.
Think about your actual routine - near the office, near a BTS station you already use, or genuinely near home.

Do I need a doctor's referral in Thailand?
No. You can book an assessment with a physical therapist directly.
If you already have an X-ray or MRI report, bring it - it fills in the picture, but it isn't required to be seen.
When you should see a doctor first
Physiotherapy isn't right for every presentation. See a doctor before booking physio if you have:
- Reduced control over bladder or bowel function
- Numbness around the buttocks and genital area
- Leg weakness that has worsened noticeably over a few days
- Fever alongside back pain, or unexplained weight loss
- Back pain following major trauma such as a fall from height or a road accident
A clinic worth its fee will tell you this itself after assessing you, rather than booking you in regardless.
